DHS Assam Skill Test Notice 2018- Grade-III Document verification @ dhs.assam.gov.in : Director of Health Services, Assam has release Skill Test and verification for Pharmacist, Lab. Tech, Radiographer, Dresser, Pharmacist (Ayur). This is for information of concerned candidates who appeared in the written examination for the Grade-III posts (Pharmacist, Lab. Tech, Radiographer, Dresser, Pharmacist (Ayur) under the Directorate of Health Services, Assam as per Advertisement No. Janasanyog / D / 10846/ 17, dated — 16/12/2017 and Janasanyog / D / 14146 /17, dated – 6/3/2018 that the Roll Nos. of the selected candidates for skill test will be displayed in DHS office Website (www.dhs.assam.gov.in) from 4:00 PM onwards on 26/7/2018. Those short listed candidates are directed to appear for Skill Test and verification of all original documents etc. The venues and schedule of the Skill Test will be displayed in the same website.
List of Documents in original are to be produced during verification and a set of photocopies are to be submitted during skill test. :-
1. Admit Card.
2. Academic Qualification.
3. Age Proof Certificate.
4. Employment Exchange Registration Card.
5. Caste Certificate.
6. Domicile Certificate.
7. Person with disabilities certificate (if any)
8. Registration Cer tificate (Pharmacist, Lab. Tech, Radiographer, Pharmacist (Ayur), Dresser (where applicable)
9. And other relevant documents, etc.
स्वास्थ्य सेवाओं के निदेशक, असम ने फार्मासिस्ट, लैब के लिए कौशल परीक्षण और सत्यापन जारी किया है। टेक, रेडियोग्राफर, ड्रेसर, फार्मासिस्ट (आयु)। यह संबंधित उम्मीदवारों की जानकारी के लिए है जो ग्रेड -3 के लिए लिखित परीक्षा में शामिल हुए
मूल रूप से दस्तावेजों की सूची सत्यापन के दौरान तैयार की जानी चाहिए और कौशल परीक्षण के दौरान फोटोकॉपी का एक सेट प्रस्तुत किया जाना है। : –
